The practice has an open list and is registering new patients whether new to the area or not.
You can register online or by downloading a paper form and bringing it to reception.
Patients applying for inclusion on the practice list are given an appointment with the Healthcare Assistant or Practice Nurse.
When the initial appointment is made patients are given forms and a sample bottle, all of which are to be handed in at the time of the appointment.
Proof of identification, including current address, such as a passport,or driving license will need to be brought to the surgery prior to your appointment being booked for your new patient check.
Medical treatment is available from the date of registration. Please contact reception for further information.
We will require a completed form for every member of your household who wishes to register. The application may take a few days to complete. If you have not heard back from the surgery within 5 working days, please contact the surgery by phoning 0300 7900 167 or email mpcc.nsgpf@nhs.net.
The service is designed and run by the NHS. It aims to cut practices’ workloads and make GP registration easier for the public.
